Before, during and after hospital

Help everyone understand the caring situation around a hospital stay.

A planned or emergency admission can change routines quickly. Clear information and early discharge conversations can make the transition safer for you and the person you support.

Before admission

  • Keep an up-to-date medication and contact list.
  • Record communication, mobility, sensory and dietary needs.
  • Take details of any health and welfare power of attorney.
  • Tell the ward that you are the person’s carer and what support you normally provide.

During the stay

With the person’s consent, ask to be involved in relevant discussions. Be clear about what care you are willing and able to provide after discharge, including anything you cannot do safely.

Planning discharge

  • Ask what has changed and what warning signs to watch for.
  • Confirm medication, equipment, transport and follow-up arrangements.
  • Check that care services can actually start when needed.
  • Ask who to contact if the plan breaks down at home.

Look after yourself too

Hospital stays can be exhausting for carers. Tell staff if you are struggling, ask for a carer’s assessment, and plan some support for the first days after discharge.
