  • Together, we explore the outcomes that matter to you and the support that may help.
  • Where a formal assessment is needed, eligible needs are considered under the Care Act 2014.
  • You receive clear next steps and, where appropriate, a support plan.

How to prepare

Think about a difficult day rather than only a good one. Note what you do, how often you do it, what happens overnight, how caring affects your health and what would make the role more sustainable.

Who can request support

This service is for adults caring for another adult in the local service area. Parent carers and young carers may have different assessment routes; the team will help you find the right support.
